Special Instructions

1. All fields with an asterisk (*) are mandatory and must be completed.
2. Please use the buttons at the bottom of the page rather than the browser buttons at the top when navigating within this questionnaire. Selecting a link on the menu bar at the top will close your session, and any information that you entered will be lost.
3. After a 60-minute period of inactivity, you will receive a notice that your session has expired; to continue your session and avoid losing information, you must activate the "OK" button within 5 minutes.
4. If you do not complete the questionnaire in one session, any answers entered during this session will be lost and you will have to start over.
